What are Cosmetic Fillers?
As we age, we lose volume in our faces due to loss of subcutaneous fat and collagen. There is less buffer between the skin and the facial muscles, so crow’s feet around the eyes and smile lines become more pronounced.
Cosmetic fillers, also known as dermal fillers, are gel-like substances injected beneath the skin to restore lost volume to soft tissue, fill in wrinkles and soften creases, or enhance facial contours.

What are the Different Kinds of Cosmetic Fillers?
There are a variety of FDA-approved cosmetic fillers that cosmetic specialists use. They are categorized according to what active ingredient they are made of. Here are the most commonly used:
Hyaluronic Acid (HA)
Hyaluronic acid is a sugar found naturally occuring in our skin. One of its intrinsic qualities is its ability to hold water a thousand times its weight in water. This helps the skin keep hydrated and plump. Over time, however, the amount of naturally occurring hyaluronic acid decreases, leading to wrinkles and creases.
Hyaluronic acid cosmetic fillers come in gel form to add volume to the dermal layer of the skin. When injected into the skin, the gel plumps up sunken areas, smooth lines and minimizes shadows. Hyaluronic acid draws in water to the injected area and this is what creates the filling effect.
The results are temporary when the gel dissolves over the course of 6 months to a year.

It’s important to take note that HA fillers contain lidocaine, an anesthetic that can reduce pain and patient discomfort during treatment. However, it can cause an allergic reaction for people who are sensitive to this substance.
Calcium Hydroxylapatite (CaHA)
Calcium Hydroxylapatite is likewise a naturally occurring substance in the body, found mainly in our bones. Used as a cosmetic filler, CaHA is used to improve the appearance of moderate to severe facial wrinkles and creases, particularly the nasolabial folds.
Calcium Hydroxylapatite is also used for HIV patients to improve signs of facial fat loss.
Compared to HA fillers, CaHA fillers are thicker and its effects normally last up to 12 months for most patients.
This FDA approved cosmetic filler can come with or without lidocaine. One of the more popular brands for this is Radiesse.
Unlike the above mentioned fillers, Poly-L-lactic acid is a synthetic substance. It is, however, biocompatible, i.e. it’s not harmful to tissues of the body and biodegradable. This product is classified as a collagen stimulator. It helps improve the appearance of fine lines by helping the skin rebuild natural collagen.
Like all cosmetic fillers, Poly-L-lactic Acid comes in gel form. This gel dissipates a few days after treatment. This product is used to treat more severe facial wrinkles and effects last up to 2 years.
FDA approved Poly-L-lactic acid fillers include Sculptra® Aesthetic.
Another synthetic but biocompatible cosmetic refill product is PMMA. It has proven its safety for use as it has been used for medical applications for a long time already. The PMMA is unique because it comes in the form of a tiny ball that remains under the skin indefinitely to provide support for the outer layer of the skin. It also contains collagen to provide structure and firmness.
PMMA provides a semi-permanent solution to medium to deep wrinkles, creases and folds. It is particularly suited for nasolabial folds. It is also used to plump up thin lips or fill out pitted scars.
It may take several injections to create the volume desired and the full effects may only be apparent after three months.
One of the downsides of PMMA is that a number of injections are needed to create volume and it can take up to three months to realize the full effects. It may also be visible under the skin.
FDA approved PMMA fillers include Bellafill® (formerly known as Artefill).
